Bowery Residents' Committee in New York City is seeking a field-based case manager for the Metropolitan Apartment Program (MAP), a transitional housing initiative for clients with mental illness or dual diagnoses. You will conduct intake, develop service plans, and provide support in clients' apartments.
The role includes ADL training, money management, medication support, and coordinating medical/mental health referrals.
