Christiana Care is seeking a Community Educator/Acute Care Connector to help patients navigate the transition from hospital to the community. In this collaborative role, you’ll identify barriers to care and connect patients with resources, reinforce discharge plans and support families after leaving the hospital.
You’ll work with providers, community partners and care teams to screen for Social Drivers of Health (SDOH), arrange transportation, follow-up visits and teach-back discussions.
