Location: Newark, DE, 19713 – Onsite at Provider Offices

Work Arrangement: 100% Onsite

Duration: 3 Months, Possible Extension/Conversion

Schedule: Monday–Friday, 8:00 AM–5:00 PM EST

Pay Range: $25.00 - $30.00 Per hr. on W2.

Position Overview

We are seeking a Patient Care Advocate to support member outreach, care-gap closure, provider engagement, and quality improvement initiatives within a managed care environment. The ideal candidate will have experience in patient/member advocacy, healthcare outreach, care coordination, health education, or quality improvement, with strong customer service and communication skills.

The Patient Care Advocate will work directly with provider offices and members to identify and address barriers to care, support preventive health services, schedule appointments, provide education, and help improve HEDIS performance and overall member experience.

Key Responsibilities
  • Serve as a liaison between members, families, providers, and healthcare facilities/agencies.
  • Conduct telephonic and face-to-face member outreach to address identified care gaps.
  • Educate members regarding preventive services, chronic care needs, and available healthcare resources.
  • Work directly within provider offices to support HEDIS care-gap closure and quality improvement initiatives.
  • Assist members with scheduling physician appointments and follow-up services.
  • Identify barriers to care and coordinate solutions, including transportation and community-based resources.
  • Educate provider office staff regarding applicable HEDIS measures and ICD-10/CPT coding requirements.
  • Review provider performance information to identify opportunities for quality improvement.
  • Collaborate with Provider Relations, Customer Service, Quality, Risk Adjustment, and other internal teams to resolve member and provider issues.
  • Document all member outreach, interventions, referrals, and follow-up activities accurately.
  • Refer members to case management or disease management when appropriate.
  • Maintain confidentiality of protected health information and comply with HIPAA and organizational policies.
  • Support special quality improvement projects and other departmental initiatives as assigned.
Required Qualifications
  • Bachelor's degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or a related healthcare field, or equivalent combination of education and experience.
  • At least 2 years of experience in direct patient care, healthcare, social work, quality improvement, health coaching, or a related environment.
  • Strong customer service and communication skills.
  • Experience with patient/member outreach and care coordination.
  • Ability to work independently in a fast-paced provider-office environment.
  • Strong documentation, organizational, and problem-solving skills.
  • Ability to travel/work onsite at provider offices in Kent County and New Castle County, Delaware.
  • Valid driver's license required for field-based provider/member activities.
Preferred Qualifications
  • Experience in managed care, Medicaid, Medicare Advantage, or dual-eligible populations.
  • HEDIS and quality improvement experience.
  • Experience with care-gap closure and member engagement.
  • Knowledge of ICD-10/CPT coding and NCQA requirements.
  • LPN, RN, LMSW, LSW, or Certified Social Worker background.
  • Experience working with provider practices and healthcare organizations.
