Acute Care Nurse Practitioner or Physician Assistant, Standard Hospitalist Medicine Role
Christiana Care is currently looking for Nurse Practitioners or Physician Assistants to join Christiana Care Hospitalist Partners (CCHP), a large and established hospital-employed group of more than 150 physicians and 30 advanced practice clinicians servicing three hospital campuses (1,300 beds) in Newark, Wilmington, DE and Elkton, MD.
The selected candidate will provide clinical management to inpatients and observation patients at the Delaware campuses located in Newark and Wilmington, working in collaboration with a hospitalist physician.
Full‑time and part‑time positions are available.

- Perform admissions, cross coverage, and discharges for patients admitted under observation or inpatient status.
- Assess health status through complete physical examinations and medical histories.
- Analyze patient data and formulate individualized care plans in collaboration with the physician, nursing, and multidisciplinary team.
- Evaluate and monitor the effectiveness of the management plan.
- Prioritize patient needs and act accordingly.
- Order, interpret, and/or perform necessary diagnostic testing or labs.
- Initiate referrals to appropriate resources and services.
- Facilitate efficient patient flow through the system or discharge.
- Document promptly and accurately in the electronic health record, including histories and physicals, consults, progress notes, orders, and discharge summaries.
- Submit billing in a timely fashion as per policy.
- Educate and counsel patients and families, employing teach‑back techniques.
- Act as liaison for patients, families, and other health professionals.
- Participate in discharge planning, resource allocation, and/or family meetings.
- Contribute to the development and maintenance of best‑practice guidelines, quality and performance‑improvement projects, algorithms, and research.
- Provide outstanding customer service and support initiatives to improve patient satisfaction scores.
- Strive for the highest quality in all aspects of work performance and support onboarding of new team members.
- Continuously learn and apply new information, skills, and knowledge.
- Maintain privileges, mandatory education, and regulatory requirements as needed for the specialty area.
- Licensed or eligible for a Nurse Practitioner or Physician Assistant license in the state of Delaware.
- Acute Care NP license required.
- Prescriptive authority or eligibility for the state of Delaware.
- DEA registration required.
- ACLS certification required.
