In-Home Health - Nurse Practitioner or Physician Assistant; -Salisbury, DE

Position Overview

Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

• Help find and connect millions of people nationwide to the right care for their unique needs.
• Fulfill the specified clinician role in partnership with Signify Health.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits when appropriate.
• Provide all indicated Diagnostic & Preventive Services products, such as spirometry, DEE, etc.
• Perform a 45‑minute visit that includes medication and medical history review, physical evaluation, and diagnostic tests as ordered by the health plan.

As an in‑home clinician, your primary workplace is the private residence of health plan members. You must be able to safely perform your duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
