Licensed Practical Nurse (LPN/LVN) – Home Health / Home Care

IJN Health Systems, LLC is a skilled home health agency looking for a Licensed Practical Nurse (LPN) to join our team of strong and compassionate caregivers. Our ideal candidate is a self‑starter, motivated, and engaged.

• Assess, plan, implement, and evaluate patient care.
• Administration of medications and documentation.
• Health promotion, teaching, and training of family members.
• Wound care.
• Patient assessments and coordination of care.
• Monitor and report changes in patient symptoms or behavior.

• L.P.N, LVN or state‑specific licensed nurse credential.
• Must be at least 18 years of age.
• Previous Home Care experience preferred, and a desire to serve and care for seniors.
• Must be licensed and in good standing (LPN) in the state of Delaware.
• Minimum of six (6) months of experience working with seniors in assisted living, home health, independent living, hospital or long‑term care environments and a desire to serve and care for seniors.
• Ability to react and remain calm in difficult situations.
• Ability to demonstrate good judgment, decision‑making skills, and problem‑solving.
• Possess written and verbal skills for effective communication and a level of understanding.
• A driver's License and good access to transportation.
• CPR certification.
