As a MDS Coordinator (RN), you will play a vital role in our care team, coordinating the Minimum Data Set (MDS) process, ensuring accurate and timely data collection, and providing exceptional patient care.

• Coordinate the MDS process, ensuring accurate and timely data collection

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor patient progress, identify areas for improvement, and make adjustments to care plans as needed

• Provide education and support to patients, families, and healthcare team members

• Maintain accurate and up-to-date patient records

• Ensure compliance with regulatory requirements and company policies

• Current RN license in the state of Delaware

• Experience as a Registered Nurse
