Family Case Manager (Full Time)

Location:
New Castle, DE
- Bachelor's in Social Work or related field.
- At least 1 consecutive year of child welfare experience
- Experience working with children and families in social services context
- Organization and strong time management skills
- Valid driver's license with properly registered and insured vehicle
- Proficient in Microsoft Office
1. Assessing the family and child(ren) using the Family Strengths and Needs Guide and Child Strengths and Needs Guide while helping the family develop a plan to mitigate the safety threats when safety threats are identified.
2. Developing a Family Service Plan with the family that addresses the identified needs related to risk and safety of a caseload of up to 12 families.
3. Reviewing the progress on the Family Service Plan with the family.
4. Assisting the family in building a support network and facilitating Family Team Meetings.
5. Assisting the family in managing crisis situations and referring families to resources or services.
