Department of Health and Social Services (DHSS) seeks a Health/Human Service Case Manager II to determine MSP, SSI, Medicaid, and SNAP eligibility. You will manage a high-volume caseload, complete initial determinations, annual redeterminations, and respond to inquiries from colleagues, providers, and the public.
Essential duties include interviewing clients, developing service plans, and maintaining comprehensive records while coordinating with state agencies and vendors.
