Behavioral Health Case Manager II

POSITION Summary:

We are currently recruiting a Care Manager for the PROMISE Program. This full-time position pays $23.50 an hour and is in Newark, DE.

• Conduct comprehensive assessments of clients' strengths, needs, and goals.
• Develop, implement, and monitor individualized care plans in collaboration with clients, families, and service providers.
• Provide referrals and linkages to behavioral health, medical, housing, employment, and social service resources.
• Coordinate with treatment teams to ensure continuity of care and successful outcomes.
• Maintain accurate and timely documentation in compliance with agency and state regulations.
• Monitor client progress and adjust service plans as needed.
• Educate clients and families about available supports, treatment options, and community resources.
• Participate in interdisciplinary team meetings and case conferences.
• Respond to crises and help connect clients with emergency services when necessary.
• Additional duties as assigned.

• Excellent verbal, written, and listening communication skills.
• Knowledge of behavioral health systems, community resources, and case management best practices.
• Empathy, patience, and professional judgment when addressing client needs.

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field, required.
• Two (2) years of professional experience in case management, behavioral health, or human services, preferred.
• Valid driver's license and reliable transportation, required.

Prolonged periods of sitting at a desk and working on a computer.

Standard hours of work Monday through Friday, 8:30 am - 5:00 pm, however evening and weekend hours may be required. Travel within the community or between sites may be required.
