Christiana Care in Newark, DE is seeking an energized Social Worker to join our care management team. You will coordinate patient discharge plans, address psycho-social barriers, and work with RN Case Managers and Utilization Management to ensure appropriate care and payer coverage.

Requires Master’s in Social Work (MSW) or MSS, and Delaware licensure; acute care experience preferred; CCM certification within 18 months encouraged.
