Reentry Coordinator

Position Overview

This position, under the supervision of the Director, will assist program staff with case management administrative duties and client record keeping.

• Maintaining client records in accordance with established policies and procedures
• Coordinating, monitoring and documenting relevant issues for clients
• Providing technical support for program staff, including client intake
• Serving as a liaison between professional staff, community agencies and clients
• Participating in all mandated staff development sessions
• Assisting with community resource meetings
• Facilitating transportation for clients
• Monitoring utilization of program resources
• Contacting institutions to confirm resident departure times
• Generating program reports to track program services
• Tracking client participation
• Performing other duties as assigned by Program Personnel

• One year of related experience in an office environment

• Understand and comply with company and contract agency rules, regulations, policies and procedures
• Communicate effectively at all levels within and outside the organization
• Express ideas clearly and completely, written and oral; manage time to meet the responsibilities and duties of this position
• Give attention to detail and follow through
• Establish and maintain appropriate working relationships with co-workers and contracting agency representatives
• Execute all duties in accordance with this position description

• High School or Equivalent
