NJ Palliative Care is seeking a dedicated and experienced Advanced Practice Provider (APP) Team Lead to support and guide our growing clinical team across New Jersey. This is a key leadership position focused on ensuring exceptional, patient-centered palliative care for individuals with serious and chronic illnesses.
AboutThe Role
The APP Team Lead will provide leadership, mentorship, and oversight to APPs while maintaining a partial clinical caseload. This role requires strong clinical judgment, excellent communication skills, and an ability to support a high-performing team in a fast-growing organization.
Responsibilities- Lead, support, and mentor the APP team across assigned regions.
- Serve as the primary clinical resource for APPs, assisting with complex cases, workflow issues, and professional development.
- Conduct regular check-ins, competency assessments, and performance reviews.
- Oversee onboarding, training, and continued education for new and existing APPs.
- Collaborate with the Medical Director and interdisciplinary team to maintain consistent, high-quality palliative care practices.
- Manage scheduling, productivity monitoring, and documentation compliance.
- Support development of protocols, quality initiatives, and best practices.
- Maintain a partial clinical caseload focused on palliative care, symptom management, and goals-of-care conversations.
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in New Jersey.
- Minimum 5+ years of palliative care experience required.
- Leadership or supervisory experience strongly preferred.
- Strong communication, problem-solving, and organizational skills.
- Ability to mentor, support, and engage clinical staff.
