Job Overview

The Case Manager for the GR-HOP w/ AIDS with a trauma informed approach conducts individualized assessment of client needs to determine eligibility and enroll them into the program.

Qualifications
  • Associate's degree preferred, High School diploma with previous experience in case management accepted
  • A demonstrated interest in the delivery of social services in a healthcare setting with excellent written and verbal skills and the ability to effectively operate within a program team
  • Displays current knowledge of local HIV community services and resources with an emphasis on housing
  • Experience working with culturally and socially diverse community groups including impacted and affected HIV individuals and groups and the underserved

Essential Functions
  • Maintains a full-time caseload of eligible individuals and families
  • Routinely monitors clients to assess progress towards achieving service plan goals, offers support and guidance as necessary to ensure success
  • Promotes compliance with regular medical appointments and treatment plans
  • Provides community referrals to clients and their families that address presenting needs in addition to housing including but not limited to food insecurity, transportation, medical care, benefits, counseling, and emotional support
  • Supports individuals in applying for longer term housing assistance to help secure eventual housing independence
  • Complete required documentation, data collection, monthly reporting and attend all meetings associated with the program
  • Coordinate HOPWA services with other program case managers and onsite multidisciplinary team
  • Assess other NBIMC non-HOPWA eligible community members housing needs and provide information and referral
  • Conduct routine housing inspections to ensure program compliance
  • As appropriate provide escorts to health care providers, entitlement resources, housing management resources and other providers as assigned
  • Collaborates with all other NBIMC FTC programs and established community partners to ensure that services are fully integrated and not duplicated
  • Performs all other project related duties as assigned
