The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. The location may be the patient’s private residence or a patient’s room in an independent living facility. The position offers a flexible schedule and work environment in various settings. The Physical Therapist evaluates and treats in the most functional environment – the patient’s home – and develops individualized therapy programs to facilitate rehabilitation.
Responsibilities/Accountabilities- Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
- Adhere to department and patient treatment schedule, timely clinical and billing documentation requirements, and work‑related meetings.
- Attend and contribute to patient care, staffing conferences and related meetings.
- Make referrals to other disciplines and agencies to address the comprehensive needs of the patient.
- Consult with other rehabilitation team members to select the most appropriate therapeutic regimen consistent with the patient’s needs and capabilities.
- Adapt program and/or plan of care according to the needs of the individual patient.
- Promote a safe environment and enforce elimination of fire and safety hazards.
- Order supplies and equipment as necessary.
- Clean, maintain, identify, and report the need for repair of equipment.
- Put patient service first: ensure patients and families receive the highest quality of service in a caring and compassionate atmosphere.
- Screen, examine and evaluate patients, including history, systems review, and application of appropriate tests and measures; synthesize information and interpret findings to establish diagnosis, identify impairments, determine predicted level of improvement and time required, identify precautions/contraindications, and design a plan of care. Develop appropriate treatment goals and methods in collaboration with the patient and caregivers, implement the treatment plan, and complete all related documentation and record keeping.
- Design, fabricate, fit and/or apply adaptive devices, and train patients in their use including orthotics and prosthetics.
- Select appropriate activities suited to the individual’s physical and mental capacity, intelligence level, and interest.
- Instruct patient in skills and techniques required for participation in mobility activities and evaluate patient progress, attitudes, and behavior as related to rehabilitative potential.
- Perform routine re‑examination as needed/required to modify/progress plan of treatment.
- Incorporate health and wellness programming into treatment plans for patients.
- Perform home and community mobility assessments and provide recommendations for safe mobility in the home and community space.
- Perform other related duties as required.
- Therapeutic exercise: aerobic conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training.
