Base pay range
$95,000.00/yr - $/yr
Position SummaryThe Hospice Nurse Case Manager is a Registered Nurse who provides compassionate care to patients receiving hospice services in their homes or in long-term care facilities. This role focuses on promoting comfort, dignity, and quality of life for patients and their families. The nurse manages the patient’s care plan, provides direct nursing care, coordinates with the healthcare team, and offers emotional support at end-of-life.
Key Responsibilities- Complete initial and ongoing assessments of patients’ physical, emotional, social, and spiritual needs.
- Develop and maintain individualized care plans based on patient needs and goals.
- Provide direct nursing care, including symptom management, medication administration, and supportive care.
- Monitor and document changes in patient condition and communicate updates to the physician and team.
- Educate and support family members and caregivers in providing comfort care.
- Pronounce death when appropriate, in accordance with state law and organizational policy.
- Participate in interdisciplinary team meetings to discuss patient status and care planning.
- Communicate regularly with physicians, facility staff, caregivers, and hospice team members.
- Ensure patients have the necessary medical supplies and equipment.
- Participate in on-call rotation for evenings, nights, weekends, and holidays.
- Provide direction and oversight for aides and LPNs involved in patient care.
- Conduct supervisory visits and review care documentation.
- Participate in training, coaching, and performance feedback.
- Maintain accurate and up-to-date patient records in accordance with hospice policies and regulatory standards.
Required
- Current and valid RN license in the state of employment.
- Valid driver’s license and reliable transportation.
- At least 2 years of nursing experience, with at least 1 year in an acute care setting.
- Strong assessment, communication, and organizational skills.
- Ability to provide compassionate, patient-centered care.
Preferred
- Experience in hospice, palliative care, case management, or working with end-of-life patients.
- Knowledge of grief, loss, and family support.
Salary
: $95k-$100k with great benefits and tuition fee reimbursements
