Advanced Practice Nurse – Per Diem (HIV Clinical Practice Team)

• Performs comprehensive physical and psychosocial assessments; develops and implements plan for care in patients across the lifespan in collaboration with interdisciplinary team; performs episodic acute care visits as needed.
• Orders patient diagnostic procedures and prescribes medication within scope of practice and in accordance with the regulations of the institution and New Jersey Nurse Practice Act.
• Documents all healthcare contacts.
• Provides ongoing patient and family education related to health status, health maintenance, disease process and treatment options.
• Attends interdisciplinary patient care conferences.

• Master’s degree in nursing; graduate of an accredited School of Nursing.
• Licensed as an Advanced Practice Nurse in the State of New Jersey.
• Current CDS and DEA licenses for the state of New Jersey.
• Current BLS and ACLS from the American Heart Association.

• Knowledge of issues faced by high HIV‑prevalence populations in New Jersey.
• Experience in a clinical practice environment such as a Ryan White clinic, Feder Qualified Health Center, and/or other community health center.
