Full-Time LPN On-Call / Tuck-In Nurse
Position SummaryLife Source Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide evening hospice tuck-in visits and after-hours patient support throughout our service area. This role focuses on ensuring patient comfort, reinforcing care plans, providing symptom observation, and supporting patients and caregivers during evening hours.
The ideal candidate is organized, responsive, compassionate, and comfortable working independently in the field while collaborating closely with the RN and interdisciplinary hospice team.
Schedule- Full-Time
- Monday through Friday
- Primarily afternoon/evening hours
- Rotating holiday/weekend coverage as needed
- Perform scheduled hospice tuck-in visits to ensure patient comfort and safety before overnight hours
- Reinforce established plans of care under the supervision of the RN Case Manager and Director of Nursing
- Observe and report changes in patient condition to the supervising RN
- Provide basic nursing care, medication reinforcement, and patient/caregiver education within LPN scope of practice
- Assist with symptom monitoring and comfort-focused interventions
- Support families and caregivers with emotional support and hospice education
- Respond to routine after-hours patient needs and communicate urgent clinical concerns to the RN on-call
- Complete accurate and timely EMR documentation in accordance with agency policy and regulatory standards
- Collaborate with the interdisciplinary hospice team to ensure continuity of care
- Maintain compliance with infection control, hospice regulations, and agency policies
- This role does not perform hospice pronouncements
- This role does not independently complete admissions or comprehensive RN assessments
- All care is provided within New Jersey LPN scope of practice and under RN supervision
- Current Licensed Practical Nurse (LPN) license in the State of New Jersey
- Minimum of 1 year nursing experience required
- Hospice, home health, long-term care, or med-surg experience preferred
- Strong communication and observational skills
- Ability to work independently in the field
- Compassionate bedside manner and strong customer service skills
- Reliable transportation and valid driver’s license required
- Current CPR certification required
- Experience with hospice EMR systems (Well Sky/Consolo preferred)
- Knowledge of hospice philosophy and end-of-life care
- Comfortable supporting patients and families in home, assisted living, and facility settings
