In-Home Nurse Practitioner or Physician Assistant Part Time - Atlantic

We’re looking for a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations and help people on their journey to better health.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and any diagnostic tests ordered by the health plan (e.g., diabetic eye exams, spirometry, etc.). You’ll use an iPad and an intuitive clinical workflow for each evaluation. You are not required to prescribe medicine, order lab tests, or alter people’s current treatment regimen.

• Visit millions of people nationwide, connecting patients to the care they need.
• Collaborate with Signify Health as a clinician colleague, fulfilling the requirements of the role.
• Travel locally and within licensed states as needed, and conduct virtual visits when required.
• Provide all Signify Health services as indicated, including diagnostic and preventive services such as spirometry, DEE, and bone density screening.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).

Anticipated Weekly Hours: 29.5 hours
Pay Range: $46.03 – $99.14 per hour (base rate, varying by experience, education, geography, and other factors)
