This is a career-defining role for a highly autonomous Palliative Care Physician or Palliative Care Nurse Practitioner who is passionate about delivering compassionate, patient-centered palliative care. You will be instrumental in optimizing the quality of life for our most vulnerable members, utilizing your expertise to manage complex symptoms, drive crucial advance care planning, and ensure dignity through seamless, timely transitions to hospice care.

As

a Palliative Care Provider, you will:
  • Provide comprehensive palliative care to patients in their homes, focusing on pain and symptom management, improving quality of life, and offering emotional and spiritual support.
  • Perform in-depth assessments of patients with serious or life-limiting conditions, develop personalized care plans, and manage their medical needs.
  • Work closely with patients and their families to establish goals of care and ensure treatment aligns with their wishes, emphasizing patient autonomy and dignity.
  • Work in collaboration with a multidisciplinary team (nurses, social workers and other healthcare professionals) to ensure a holistic approach to patient care.
  • Participate in a shared on-call rotation, as directed by the Senior Medical Director.
You should get in touch if:
  • You have an MD or DO degree and are Board Certified or Board eligible in Palliative Care, Hospice & Palliative Medicine, or a related field.
  • You are excited about practicing in New Jersey, we welcome the opportunity to support relocation for exceptional physicians looking to make this region their home.
  • You are (or are willing to become) licensed in New Jersey or Georgia, with DEA and CDS registrations, we will support you in obtaining these if needed.
  • You have a minimum of 3 years of clinical experience in palliative care, hospice, or a related field. Experience in a home-based care setting and/or value based care is preferred.
  • You are based in the state of New Jersey or Georgia.
  • You possess a BSN from an accredited program and a Master's degree in advanced clinical practice nursing or a post-Master’s certificate.
  • You hold an active Nurse Practitioner license to practice in New Jersey, an active DEA registration, and an active CDS registration.
  • You have prior experience working as part of an in-patient palliative care team and/or as a hospice RN.
  • You have a history of working in a house call setting, and are board certified in Palliative Care or Hospice Nursing (preferred).
