Job OverviewComprehensive care coordination for patients—including initiation and coordination of incoming and outgoing transfers for higher level of care. Responsibilities include securing physician specialist acceptance, clinical assessment, triage, bed placement, insurance authorizations, and arranging patient transportation. Coordinates patient placement among multiple network facilities for admissions and transfers, prioritizing transfers based on acuity.
Responsibilities- Coordinate patient placement and safe transfers among multiple network facilities.
- Perform patient registration, bed placement requests, and insurance authorizations.
- Conduct clinical assessments and triage to determine appropriate level of care.
- Arrange transportation resources and manage transport schedules.
- Maintain communication with internal and external customers, including physicians, nurses, and transportation agencies.
- Document all transfers accurately in the Central Logic and other relevant systems.
- Monitor and improve patient throughput to optimize outcomes and efficiencies.
- Ensure compliance with EMTALA, HIPAA, and institutional policies.
- Participate in interdisciplinary teams to improve patient flow and care processes.
- Support training and education of staff and community partners.
- Active BSN RN license in the state of practice, temporary RN license, or compact RN licensure.
- Critical Care experience (ALS/PALS/BLS) with at least three years in the care of the population served.
- Experience in clinical care operations, case management, and patient throughput.
- Working knowledge of hospital operations and community resources.
- Demonstrated critical thinking, conflict resolution, time management, and problem‑solving skills.
- Excellent communication skills and ability to collaborate across departments, facilities, and organizations.
- Proficiency with Microsoft Office and ability to type at least 40 words per minute.
- Strong organizational and interpersonal skills to maintain rapport with internal and external customers.
- Active BSN RN license in state of practice, temporary RN license, or compact RN licensure.
- Provide 10 shifts of availability per calendar quarter, submitted by the 15th of the previous month.
- Must work at least 6 of the submitted 10 shifts.
- 12‑hour shifts required.
- Evaluate medical necessity and appropriateness of care, optimizing patient outcomes.
- Facilitate patient throughput to final destination with quality focus.
- Coordinate patient placement among multiple outreach and network facilities.
- Serve as liaison with other departments, leaders, and physicians to improve communication and patient flow.
- Manage patient data in Central Logic, telephone systems, and registration systems.
- Maintain regulatory compliance (EMTALA, HIPAA, quality initiatives).
- Represent the organization in external community and professional settings.
- Provide consultant input on managed care integration.
