Hospice RN Regional Manager

Grace Healthcare Services is seeking a compassionate and experienced nurse to lead and coordinate hospice care across multiple regions.

• Lead Quality Hospice Care: Collaborate with the Clinical Director to develop, implement, and evaluate hospice care programs, ensuring compliance with state and federal regulations.
• Supervise Interdisciplinary Teams: Oversee the interdisciplinary team (IDT), including orientation, scheduling, education, and support of all caregivers, ensuring cohesive and comprehensive patient care.
• Ensure Regulatory Compliance: Maintain an up-to-date understanding of hospice state licensure and federal regulations, and monitor services and documentation to ensure adherence to policies.
• Foster Communication: Act as liaison between the Clinical Director, IDT members, patients, families, and customers, facilitating effective communication to resolve concerns.
• Develop Physician Relationships: Build and nurture relationships with physicians to enhance collaboration and patient outcomes.
• Participate in On-Call Support: Provide on‑call support as needed to ensure continuous patient care.
• Drive Business Development: Engage in business development activities, including implementations and presentations, to promote hospice services and expand reach.

• Valid NJ or Multi‑State RN License.
• Bachelor's or Associate degree in nursing preferred.
• At least one year of hospice or home‑care experience with progressive management responsibilities.
• Excellent communication and interpersonal skills.
• Willingness to travel across multiple RWJ locations.
