Licensed Practical Nurse (LPN)

Seeking a licensed practical nurse (LPN) to provide skilled nursing care in a long-term care setting. The ideal candidate will possess experience in managing patients with chronic conditions and working within a busy long-term care unit.

Responsibilities:

• Administer medication and treatments according to physician orders and facility protocols.
• Monitor patient health status and report changes to the nursing team.
• Document patient care activities accurately in electronic health records.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Ensure compliance with regulatory standards and safety protocols.
• Provide compassionate patient care, emphasizing dignity and respect.

Qualifications:

• Valid NY license to practice as an LPN.
• Minimum of 1 year of recent experience in a LTC/SNF setting.
• Proficient in EPIC electronic health record system.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and as part of a team.
• Must meet travel compliance and background policy requirements.
