Job SummaryAs an In-home Intercept Case Manager, you will provide individualized family sessions in the home and community settings to maintain safety for youth in the home. You will manage a caseload of about 5 families, providing an average of three face‑to‑face sessions per week to divert youth from out‑of‑home placements and strengthen the family unit.
What You Will Do- Provide individualized family sessions in the home and community.
- Carry a small caseload of five families.
- Meet, on average, three times face‑to‑face with the families each week to provide support and strengthen the family unit.
- Attend three required meetings per week (group supervision, clinical consultation, and individual development) for program model adherence, case conceptualization, and professional development.
- Complete all documentation related to families, including bi‑weekly treatment plans, weekly session notes, and ongoing clinical assessments.
- Advocate for the families in any involved system, such as CP&P, gathering information specific to intervention development.
- Work closely with DCP&P staff to assist families in addressing needs and strengthening the family system.
- Provide after‑hours support to families using a rotating on‑call schedule and respond in‑person to some crisis situations as deemed necessary.
- Master's Degree in social work, psychology, or related field.
- Or Bachelor's degree in social services with one-year related experience in counseling or case management.
- A valid driver's license insurable by the agency's automobile insurance.
