The Case Manager at The Apostle's House is responsible for providing comprehensive support to individuals and families experiencing homelessness. This role involves assessing clients’ needs, developing individualized service plans, connecting them to necessary resources, and supporting them on their path to stable housing and self-sufficiency. The Case Manager will work collaboratively with other staff members, community partners, and social services agencies to ensure each client’s holistic needs are addressed.

Key Responsibilities:

Client Intake & Assessment
:

Conduct thorough intake interviews to assess each client’s housing, financial, mental health, and social service needs.
Develop individualized service plans (ISPs) based on client assessments, outlining short- and long-term goals.

Case Management & Advocacy
:

Provide ongoing case management support to clients by monitoring progress, adjusting ISPs, and offering guidance.
Advocate on behalf of clients to access housing assistance, employment opportunities, healthcare, mental health services, and other necessary resources.
Coordinate referrals to appropriate community partners or government agencies to address clients’ specific needs.

Resource Navigation
:

Assist clients in applying for and securing housing, public benefits (such as SNAP, Medicaid, SSI/SSDI), and other social services.
Support clients in developing life skills such as budgeting, job search strategies, and household management.
Connect clients with job training, educational programs, and employment services.

Crisis Intervention & Conflict Resolution
:

Provide crisis intervention services as needed, including de‑escalation, conflict resolution, and emotional support.
Collaborate with mental health professionals and social service agencies to address any behavioral health crises.

Documentation & Reporting
:

Maintain accurate and up‑to‑date client files, including case notes, service plans, and progress reports.
Ensure timely documentation of all services provided and outcomes achieved in compliance with shelter policies and grant requirements.
Attend monthly supervision to progress note monitor.

Collaboration & Communication
:

Work closely with shelter staff, social workers, healthcare providers, and other stakeholders to coordinate comprehensive client care.
Attend team meetings, case conferences, and staff training sessions to share updates on client progress and best practices.
Develop and maintain strong relationships with community partners to enhance service delivery.

Follow-Up & Aftercare
:

Conduct follow‑up services to ensure successful transitions for clients who have moved into stable housing or other long‑term solutions.
Provide ongoing support to clients to help them maintain stability in their housing and employment.

Qualifications:

Education
:
Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (Master’s degree preferred).
Experience
:
Minimum of 2 years of experience in case management or working with homeless or at‑risk populations.
Skills
:
- Strong interpersonal, communication, and advocacy skills.
- Ability to manage multiple cases with varying degrees of complexity.
- Knowledge of community resources, housing programs, and social service systems.
- Problem‑solving, crisis intervention, and conflict resolution skills.
- Proficiency in Microsoft Office and case management software.
Certifications
: CPR, First Aid, or other crisis management certifications (preferred).
Other Requirements
:
Must pass a background check and possess a valid driver’s license.

Working Conditions:

The Case Manager will work primarily at the homeless shelter but may be required to travel to community agencies or client housing.
Flexibility in work hours may be needed, including occasional evenings and weekends.
