We are seeking a detail-oriented and compassionate MDS professional to join our skilled nursing facility team in Ohio. The ideal candidate will be an RN or LPN with experience in MDS 3.0 assessments, care planning, and PDPM reimbursement processes. This is a critical role that directly supports our commitment to providing high-quality care and achieving positive resident outcomes.

• Accurately complete and submit MDS assessments in accordance with federal and state guidelines.
• Manage the RAI process and ensure timely completion of all assessments and care plans.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Monitor and ensure compliance with Medicare, Medicaid, and PDPM regulations.
• Provide education and guidance to nursing staff on MDS processes and best practices.
• Participate in Quality Assurance and Performance Improvement (QAPI) activities.

• Current Ohio RN or LPN license in good standing.
• Minimum of 1 year experience in MDS coordination within a long-term care or skilled nursing environment.
• Strong knowledge of MDS 3.0, PDPM, and CMS regulatory requirements.
• Excellent organizational, communication, and leadership skills.
• Experience with electronic health record (EHR) systems (Matrix Care, Point Click Care , etc.) preferred.
