Responsibilities
- Help adults with developmental disabilities discover, pursue and achieve what's important to them
- Assess needs to determine desired outcomes
- Coordinate services and refer to community resources
- Create and implement an Individual Service Plan (ISP) for adults with developmental disabilities
- Minimum of an Associate's Degree
- Bachelor's Degree preferred
- Must be well organized
- Detail oriented
- Ability to meet deadlines
- Passion and enthusiasm to make a difference in the lives of adults with developmental disabilities
