CDCA Case Manager

Job Summary: Provide support services and foster community by assessing the needs of their clients and developing and implementing a comprehensive plan to address those needs. This position reports to a Case Management Supervisor.

• Coordinatesandprovidessafe,timely,effective,efficient,equitable,culturally sensitive,andclient- centered care.
• ForSUDClients:
  
  Handlescaseassignments,assessesbarrierstorecoveryinthevariouslifedomains, may draft SUD case management-specific service plan, review progress in case management goals.
• ForMHClients:
  
  Handlescaseassignments,assessesbarrierstorecoveryinthevariouslifedomains, coordinatesmentalhealthtreatmentplanningincludinggoalsforCPSTservices,provides input,and works collaboratively with the clinical counselor, psychiatrist, and/or nurse practitioner in the development of the treatment plan.
• Isfamiliarwiththediagnosticassessmentcompletedfortheclientsserved,whichincludesareviewof diagnosis, identification of current symptoms, risk, functioning,identified needs in the client's life domains shown as Z-Codes todetermineeligibility for Case Management or CPST services and the level of care needed.
• Workscollaborativelyfollowingupwithobservationswiththeclinicalcounselor,psychiatrist,and/or nurse practitioner in the development of the treatment plan.
• Adheresto Lighthouse'sprotocolforappropriate"care management"activitiesaspartofthecontinuum of services to meet each client's defined needs and as defined in the treatment plan.
• Provides follow-up and appropriate referrals to community resources are essential. Collaboration and coordinationofservicestoensurecontinuityofcareandefficiencyofservicesacrosssystemsisvitalto early recovery.
• Provideslinkageandassistanceinaccessinginpatientormorerestrictivetreatment,asmedicallynecessary.
• Staysabreastofbestpracticesinmodalitiesofinterventiontomaintainhigh-qualitycaremanagement that is medically necessary to client specific treatment plan.
• Evaluatesdailylivingfunctioningandassistclientsincreatinggoalstoimprovedailyliving.
• Completesbillinganddocumentationaccuratelyinatimelymanner.
• Seesclientsweeklyorasclinicallyappropriate according to theclient’slevelofcare.
• EnsuresallcommunityactivitiescomplywithHIPAA,42
  
  CFRPart2,andMental
  
  Healthlawsand regulations on confidentiality of PHI.
• Keepsabreastofcommunityresourcesandmakesneededreferralstomeetclientneedsonatimelybasis.
• Coordinatesandcollaborateswithothercommunityagenciesregardingclient'streatmenttopromote positive treatment outcomes. Participate in community collaboration meetings.

Required Qualifications:

• CDCA certification
• Must pass BCI check, all Corporate Compliance checks, and employment drug screen

Qualifications:

• Knowledge of Job and Family Services procedures
• Bachelor'sDegreein Social Work ,Psychology,Counseling,oradirectlyrelatedhumanservicefield.
• 2 years relevant experience in case management, social services, or a related field

Schedule
:
Monday - Friday 8:30am - 5:00pm

Compensation range
: $20.19 - $26.60
