This is a full-time, on-site Social Worker role based in Newark, OH. The Social Worker provides psychosocial assessments, counseling, and support to patients and families facing serious illness and end-of-life transitions. Daily responsibilities include developing individualized care plans, coordinating community resources, and collaborating with an interdisciplinary hospice team that may include nurses, physicians, chaplains, and volunteers. The role involves advocacy for patient and family needs, assistance with advance care planning and end-of-life decision-making, and documentation of services in accordance with regulatory and organizational standards.

The Social Worker also participates in family meetings, bereavement support, and education for patients, caregivers, and the community about hospice services.

• Relevant social work skills including psychosocial assessment, counseling, crisis intervention, and care planning.
• Strong communication, active listening, and interpersonal skills for working with patients, families, and interdisciplinary teams.
• Ability to coordinate community resources, provide patient and family advocacy, and navigate healthcare and social service systems.
• Organizational and documentation skills, including maintaining accurate records and meeting regulatory and compliance requirements.
• Demonstrated compassion, cultural humility, and sensitivity to diverse backgrounds, beliefs, and family structures.
• Master's or Bachelor's degree in Social Work from an accredited program; state social work licensure (or eligibility) as required in Ohio.
• Experience in hospice, palliative care, healthcare, or related settings is preferred.
• Ability to work on-site in Newark, OH, with reliable transportation and flexibility to meet patient and family needs.
