Under the direction of collaborating physicians, delivers health and medical care within the scope of practice of the nurse practitioner as defined by the Advanced Nurse Practice Act. Job requires knowledge, skill and ability to provide care to an adult patient population.

• Obtains medical, family and social histories from patients.
• Assesses patient and family needs.
• Performs physical examinations. Recognizes deviations from normal in physical assessment and works in collaboration with the physician to develop necessary management and treatment plans.
• Refers patients for diagnostics, surgical and psychological services.
• Authenticates findings, reports and plans in the medical record with appropriate detail.
• Provides patient education opportunities for individual and/or family utilizing additional resources when necessary.
• Facilitates patient participation in health care by providing information needed to make decisions about promotion, maintenance and restoration of health and appropriate utilization of health care resources.
• Participates as an interdisciplinary team member to provide coordinated care with collaborating health care providers and agencies.
• Contributes to health education of individuals and groups through outreach programs.
• Evaluates effectiveness and efficiency of care and participates in quality improvement activities as a team member to improve the value of care provided.
• Understands and provides leadership in the quality and utilization management processes.
• Pursues professional growth through continuing education and sharing experiences with colleagues.
• Ensures personal performance aligns with Mission, Vision, Standards of Behavior and Service Goals.
• Maintains professional readiness for emergencies and adheres to relevant clinical standards.

• NP experience preferred
• Graduate of accredited nursing program
• Maintains current licensure in Ohio
• Functions within the scope of practice of nursing for nurse practitioners as defined by the Advanced Nurse Practice Act
• Completed an approved nurse practitioner program (masters-prepared)
• Certified as a nurse practitioner by a nationally recognized certifying organization
• Demonstrates clinical skill and judgment within the NP scope of practice
• Demonstrates ability to provide care for adult patients under physician supervision
• Strong communication skills with patients, families and healthcare team
• Professional flexibility and adaptability in a collaborative environment
• Commitment to continuing education and improving health outcomes
• Maintains BCLS and ACLS as applicable
• Proficiency with computer systems (Epic or equivalent)
