• Active Licensed Practical Nurse (LPN) license in the state of Ohio
• Minimum 1 year of long-term care experience required
• Current CPR certification preferred
• Ability to perform physical job duties
• Ability to pass background screening requirements

• Monitor patient vitals and document accurately
• Administer medications and treatments as ordered
• Assist nursing and medical staff with patient care needs
• Evaluate patient condition and report changes to supervising staff
• Educate patients and families regarding treatments and care plans
• Perform wound care including dressing and bandage changes
• Maintain patient comfort, safety, and cleanliness
