We are seeking a compassionate and skilled Family Nurse Practitioner to join our private primary care practice. This is a full-time, on-site role located in New Albany, OH. The Family Nurse Practitioner will be responsible for providing high-quality primary care services, including routine check-ups, diagnosis, treatment plans, and management of chronic conditions. Additional responsibilities include collaborating with the healthcare team to address urgent care needs, promoting preventive care, and ensuring a positive patient experience.
- Full-time position
- 4.5 days in office per week
- Established patient base and strong clinical support staff
- Collaborative team environment with experienced physicians and APPs
- Provide comprehensive outpatient primary care services to patients across the lifespan
- Manage chronic conditions including hypertension, diabetes, hyperlipidemia, thyroid disease, and other common primary care concerns
- Evaluate and treat acute illnesses and same-day sick visits
- Perform annual wellness visits and preventive care screenings
- Conduct hospital follow-up visits and transitional care management
- Review and manage laboratory results, imaging, and ancillary testing
- Respond to and manage EHR messages, medication refills, and patient communications
- Collaborate closely with physicians, nursing staff, and clinical support team
- Participate in occasional call coverage (approximately 2–3 days per month)
- Board certified and valid licensed as Family Nurse Practitioner in good standing within the state of Ohio
- Strong clinical judgment and communication skills
- Ability to work independently while collaborating within a team-based care model
- Prior primary care experience preferred, but motivated new graduates are encouraged to apply
- Competitive salary: $95,000–$110,000 annually, based on experience
