Position OverviewOhio’s Hospice is seeking a compassionate Registered Nurse (RN) to join our Hospice House/Inpatient Unit team on a PRN basis. This overnight position is ideal for nurses who are passionate about providing exceptional end-of-life care in a collaborative and supportive environment.
Schedule
- PRN
- Night shift: 6:30 p.m. – 7:00 a.m.
- Every third weekend rotation required

Key Responsibilities- Deliver comprehensive nursing care using the nursing process, including assessment, planning, implementation, and evaluation.
- Perform ongoing assessments of patients’ physical, emotional, psychosocial, and spiritual needs.
- Develop individualized care plans with measurable goals in partnership with patients, families, and the interdisciplinary team.
- Regularly review and revise care plans, medications, interventions, and visit frequency based on changing patient needs.
- Monitor patient status, communicate significant changes to the physician, and assist in determining the appropriate level of hospice services.
- Participate in hospice admissions, interdisciplinary team (IDT) meetings, and care conferences to ensure coordinated, high-quality care.
- Accurately document physician orders, patient assessments, interventions, and clinical updates in the electronic health record.
- Provide guidance, education, and oversight to Personal Care Specialists, volunteers, and other caregivers to ensure care aligns with the established plan.
- Complete documentation promptly and according to Ohio’s Hospice standards to promote continuity of care.
- Communicate patient updates effectively during shift handoffs using SBAR or other approved communication tools.
- Support other departments and clinical teams as needed to ensure patients receive seamless, comprehensive care.
- Participate in quality improvement initiatives that strengthen patient outcomes and organizational performance.
- Assist with onboarding and mentoring newly hired clinical staff as assigned.
- Graduate of an accredited Registered Nursing program; BSN preferred.
- Current, unrestricted Registered Nurse (RN) license in the State of Ohio.
- Minimum of two years of acute care nursing experience preferred.
- Hospice and Palliative Nurse Certification (CHPN) preferred. Ohio’s Hospice supports eligible employees who wish to pursue certification.
- Current CPR certification.
- Proficient computer skills with the ability to document accurately in an electronic medical record.
- Valid driver’s license with a safe driving record.
- Current automobile insurance that meets state minimum requirements.
- Ability to travel in varying weather and driving conditions when needed.
