This role is for Licensed Practical Nurses (LPN) providing homecare visits, including medication administration, insulin assistance, and wound care, primarily during evening and weekend hours.
Responsibilities involve planning, implementing, and evaluating patient care using the nursing process, administering treatments, monitoring patient status, and coordinating care with physicians.
Qualifications include a current LPN license, health certificate, PPD or chest X-ray, and BLS certification.
Key duties encompass patient assessment, medication management, care documentation, and patient education.
There are different team tiers:
Tier 1 requiring availability for 4 days/week with at least 32 visits, and Tier 2 requiring 3 days/week with a minimum of 24 visits. A casual/PRN option is also available without set availability.
