LPN Homecare Visits

We are seeking Licensed Practical Nurses (LPNs) to provide homecare services for adult patients, performing quick visits that include medication administration, insulin support, wound care, and patient monitoring.

Responsibilities: Utilize the nursing process to plan, implement, and evaluate patient care, initiate and revise care plans, administer prescribed treatments, respond to patient condition changes, and provide healthcare education and discharge planning.

Qualifications: Valid LPN or RN license for the practicing state, current health certificate, PPD or chest X-ray, and a current BLS card.

Skills: Strong clinical judgment, ability to respond promptly to patient needs, and effective communication skills.

Key Duties: Conduct patient assessments, follow care protocols, document interventions, and collaborate with healthcare teams to ensure quality care.
