What

You’ll Do

As a Resident Care Coordinator, you’ll coordinate and oversee bedside care within assigned Health Services neighborhoods. You’ll provide direct supervision to care staff while ensuring residents receive care that meets the highest organizational and regulatory standards.

Leadership & Supervision
  • Directly supervise resident care staff: assignments, performance monitoring, evaluations, coaching, and documentation
  • Partner with the Resident Care Manager (LN) on resident evaluations and service plan updates
  • Assist in hiring, training, disciplining, and supporting care staff
  • Help plan and deliver staff trainings and in‑services
  • Participate in on‑call/standby rotation and assist with staffing coordination
Resident Care & Advocacy
  • Monitor residents’ health status and report significant changes
  • Support and oversee medication processes with med‑techs (including triple‑check safety standards)
  • Ensure accurate documentation of nursing services
  • Provide ADL assistance as needed
  • Initiate preventative and rehabilitative nursing procedures
  • Investigate incidents and implement timely corrective actions
Communication & Collaboration
  • Communicate with families regarding resident needs
  • Coordinate physician, dental, and eye appointments
  • Collaborate with pharmacists, providers, home health, and leadership teams
  • Promote positive community relations
Compliance & Quality
  • Ensure compliance with state regulations and licensing requirements
  • Follow HIPAA privacy standards and facility policies
  • Monitor environmental safety hazards
  • Assist with quality assurance initiatives
What You Bring Required:
  • Certified Nursing Assistant (CNA) certification in Oregon or relevant unlicensed caregiving experience (2+ years preferred)
  • Knowledge of care/service plans, medication systems, infection control, nurse delegation, and residents’ rights
  • Ability to pass DHS background check, drug screen, and reference checks
  • Valid driver’s license (if required for role)
  • Authorization to work in the U.S.
