Direct Support Crisis Specialist

• Provide direct care and support in a 24-hour, seven-days-per-week residential group home
• Support children and adults with behavioral supports, safety, personal care, nutrition, housekeeping, health care, and daily activities
• Honor individuals’ wishes and culture while maintaining health and safety
• Assist individuals with community access and participation in educational and recreational activities
• Observe and document individuals’ activities, behavior, and progress
• Administer medications and treatments under nursing and medical direction
• Assist with activities of daily living and hygiene, including personal care, bathing, toileting, and feeding
• Document and track all aspects of care, answer phones, and maintain confidentiality
• Prepare meals and complete housekeeping tasks
• Support safe transitions back to individuals’ home communities

• Eighteen months of experience providing direct individual care or training in a related human services field, or an Associate or higher-level degree in a related human services field, or an equivalent combination
• Valid driver’s license and acceptable driving record
• Experience following written and verbal instructions
• Experience communicating clearly in writing and with diverse individuals
• Experience resolving conflicts professionally and promptly
• Experience using computers, tablets, and cell phones for documentation, reporting, and communication, including Internet, Microsoft Office, and Therap
• Experience following behavioral, mental health, and/or medical supports and orders
• Experience monitoring or assisting individuals with mental, emotional, behavioral, physical, or medical challenges
• Ability to work days, evenings, nights, weekends, holidays, and occasional overtime
• Ability to work at nearby worksites when transferred
• Ability to lift up to 75 pounds and frequently squat, bend, and stoop
• Ability to engage and maintain Oregon Intervention System-approved restraints for one hour or less
• Ability to complete a three-week in-person training and obtain First Aid/CPR, Oregon Intervention System, and Medication Administration certifications
• Must pass criminal history and background checks and drug testing
• Must be authorized to work in the United States; the employer does not offer visa sponsorship
• Must meet ODHS SACU Driving Policy requirements
• Must complete DHS Form I-9 and E-Verify employment authorization confirmation

Demonstrates expertise in providing direct care and support to individuals with diverse needs, including behavioral and medical challenges. Proficient in documentation, communication, and adherence to safety and health protocols.
