, you'll play a key role in helping older adults and their families navigate one of life's most important transitions with confidence, compassion, and dignity.
This isn't a traditional floor nursing role. You'll be the clinical coordinator behind seamless admissions, working closely with residents, families, providers, and interdisciplinary teams to ensure every new resident receives a safe, organized, and welcoming start in our Residential Care community.

What You'll DoAs the primary clinical contact for admissions, you'll:
- Coordinate new admissions, readmissions, and short-stay resident transitions.
- Review clinical referrals, medical records, and admission documentation.
- Complete nursing assessments and support person-centered care planning.
- Conduct medication and treatment order reviews and assist with medication reconciliation.
- Partner with residents, families, physicians, hospitals, therapists, and community providers.
- Ensure all admission documentation is accurate, complete, and compliant.
- Help residents and families feel informed, supported, and welcomed.
- Collaborate with nursing, caregiving, therapy, and administrative teams to create smooth transitions into care.
- Support continuity of care for Friendsview Life Plan members utilizing short-stay or recovery services.
- Follow up with new residents to ensure a positive admission experience.
- Build meaningful relationships instead of rushing from task to task.
- Focus on coordination, communication, and resident experience.
- Work with a collaborative interdisciplinary team.
- Help shape positive outcomes from day one.

- Current CPR/BLS certification (or ability to obtain)
- Strong clinical assessment and documentation skills.
- Excellent communication and organizational abilities.
- Ability to manage multiple priorities while maintaining attention to detail.
- Commitment to person-centered care and exceptional customer service.
- Experience in assisted living, residential care, skilled nursing, rehabilitation, home health, hospice, or senior living.
- Experience with admissions, transitions of care, discharge planning, or medication reconciliation.
- Familiarity with electronic health records (EHRs).
- Knowledge of Oregon Residential Care Facility regulations.
- Compassionate and relationship-driven.

- Comfortable coordinating across multiple teams.
- A strong communicator who can put residents and families at ease.
