Senior Transitional Planner

Reentry Works seeks to improve transitional planning and reentry outcomes for individuals leaving state incarceration by assessing their needs and connecting them with appropriate services prior to their release from NYS prison. This role would work out of our Newburgh office but would require travel to prisons within approximately 50 miles of the Newburgh office.

We are seeking a full-time Senior Transitional Planner to join our Reentry Works unit. This position will supervise a small team of one or two Transitional Planners as they create transitional plans for individuals who are being released from The Department of Corrections and Community Service (DOCCS) custody to New York City. The Senior Transitional Planner carries a small caseload of their own as well.

This position requires clearance by the New York State Department of Correction and Community Supervision (DOCCS). Candidates must be able to obtain and maintain Department of Correction and Community Supervision (DOCCS) clearance to work in facilities operated by the Department of Correction and Community Supervision (DOCCS).

Salary: $34.62-$35.85 an hour

Essential Duties:
  • Supervise Transitional Planners who develop transitional plans based on individuals' criminogenic risk and needs, including a wide variety of challenges such as homelessness, substance misuse, and unemployment
  • Guide Transitional Planners on providing programs, resources, and referrals to participants during their re-entry
  • Oversight of service deliveries in prisons, referral coordination, and Liberation Day transportation
  • Collaborate with the Program Coordinator and Program Director to ensure all program goals and objectives are being met
  • Analyze, interpret, and input required data into record-keeping systems
  • Travel to New York State prisons within approximately 50 miles of the Newburgh Office as needed
  • May assist with other grants as needed
  • Attend assigned trainings
  • In-person attendance is required to collaborate with co-workers
  • Perform other duties as assigned
Minimum Qualifications:
  • Associate's Degree in Human Services or a related field with two years of related experience in a social service organization; in lieu of a degree, High School Diploma, or equivalency with three years of related experience in a social service organization
  • Valid New York State driver's license
  • Bilingual (Spanish) is preferred
