Job Summary

We are seeking a highly skilled and compassionate Subacute Charge Nurse to join our team at Port Rehabilitation and Healthcare Center. As a Subacute Charge Nurse, you will play a crucial role in providing high-quality, patient-centered care to our residents. In this full‑time position, you will be responsible for supervising and supporting a team of healthcare professionals, coordinating patient care, and ensuring regulatory compliance.

Key Responsibilities
  • Clinical Oversight:
    Supervise and support RNs, LPNs, and CNAs to ensure safe, effective care delivery.
  • Resident Care Management:
    Perform assessments, administer medications and treatments, monitor changes in condition, and respond to emergencies.
  • Care Planning:
    Collaborate with physicians, therapists, and interdisciplinary team members to develop and update individualized care plans.
  • Regulatory Compliance:
    Ensure adherence to state and federal regulations, facility policies, infection control standards, and documentation requirements.
  • Admissions & Discharges:
    Coordinate admissions, transfers, and discharges, ensuring continuity of care.
  • Communication:
    Serve as the primary point of contact for residents, families, and providers regarding clinical concerns.
  • Staff Leadership:
    Assign duties, provide guidance, mentor staff, and address performance or workflow issues during the shift.
  • Quality & Safety:
    Promote fall prevention, wound care management, medication safety, and infection prevention initiatives.
Qualifications
  • Active RN or LPN license (as required by state).
  • Previous experience in skilled nursing, long‑term care, or subacute rehabilitation preferred.
  • Strong leadership, clinical judgment, and organizational skills.
  • Ability to manage multiple priorities in a high‑acuity, fast‑paced setting.
