Registered Nurse (RN) Case Manager

Location:

Newburyport, MA 01950 Job Type: Full-Time

Schedule:

Monday-Friday, 8:30 AM - 4:30 PM, with occasional weekend phone support as needed.

Position Overview

We are seeking a compassionate and organized Registered Nurse (RN) Case Manager to support adults with developmental and intellectual disabilities residing in residential group homes throughout the North Shore region of Massachusetts. This role is primarily focused on healthcare coordination, case management, and clinical oversight rather than direct bedside nursing.

The RN Case Manager will serve as a liaison between healthcare providers, residential staff, families, and program leadership to ensure individuals receive comprehensive, coordinated, and high-quality care.

Key Responsibilities
  • Coordinate and manage healthcare services for individuals in residential settings.
  • Schedule and monitor primary care, specialty, hospital, and community-based appointments.
  • Review medical records, physician recommendations, and clinical documentation.
  • Assess healthcare needs and support individualized care planning.
  • Collaborate with physicians, specialists, pharmacies, and healthcare providers.
  • Provide medical guidance and training to residential staff.
  • Monitor health outcomes and ensure timely follow-up on medical recommendations.
  • Maintain accurate documentation and regulatory compliance.
  • Travel to assigned residential locations for assessments and care coordination activities.
Qualifications Required
  • Active Registered Nurse (RN) license in Massachusetts.
  • Experience in case management, care coordination, or community-based nursing.
  • Strong organizational, communication, and documentation skills.
  • Ability to work independently and manage multiple priorities.
  • Reliable transportation and willingness to travel locally between residential sites.
  • Experience working with individuals with developmental or intellectual disabilities preferred.
  • Knowledge of Medication Administration Program (MAP) guidelines preferred; training available if needed.
