Clinical Team Manager

Responsibilities- Oversee all patient care services and personnel, including patient and staff assignments, referral coordination, continuous needs assessment, and development, implementation, and updates of individualized patient plans of care.
- Plan, direct, and participate in the delivery of home health services to provide diagnosis‑specific patient care, coordinating services with providers, vendors, or facilities.
- Ensure timely start of patient care within regulatory guidelines.
- Process and coordinate physician orders, staff assignments, and communications among direct‑care team members, patients, families, and physicians.
- Review, approve, and evaluate plan‑of‑care changes for clinical appropriateness.
- Integrate orders from all relevant physicians and confirm approval by responsible physician.
- Conduct regular OASIS reviews for all patients.
- Meet regularly with direct‑care team members to provide guidance, coaching, and communicate information about specific issues or organizational changes.
- Initiate hiring requests, interview candidates, and recommend hiring of direct‑care staff.
- Communicate frequently with the Branch Director or Administrator on key performance metrics.
- Remain available for field work as needed.
- Perform other duties or projects as assigned.
- Associate degree in a nursing‑related field required.
- Two years of home care experience as a registered nurse in a home health or hospice environment.
- One year of supervisory experience preferred.
- Current, unrestricted RN license valid in the state of work.
- Proficiency with enterprise applications such as Home Care Homebase, Workday, and basic Microsoft Office skills.
- Willingness and ability to travel within branch coverage area; must have a dependable vehicle, valid driver’s license, and current auto insurance.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
Base salary: $95,000 to $110,000 per year. Salary is determined by education, experience, skills, and internal equity. The role may also qualify for performance‑based bonuses, commissions, on‑call pay, or other incentive earnings.
