Permanency Social Worker
Permanency Social Worker accomplishes the following:
Has face to face contact with families during supervised visitation. Conducts the supervised visit to ensure the safety and success of each visit. These supervised visits are based on the referrals sent to Community Residences from DCF. The Permanency social worker will assist the family to ensure children actively visit their family members and maintain the parent child bond for children removed from the home.
Transports child to supervised visit and their foster care location before and after visits. A car will be provided by Community Residences Inc.
Provides the family with feedback on their parenting to help learn the techniques for a positive visit.
Maintains a running account of billable hours worked on each case. Submit bills to DCF monthly.
Interacts and relays information to DCF worker both by phone and written reports following the visit.
Responsible for overseeing the contract with DCF and ensuring the contract is in compliance.
Attends monthly clinical supervision and quarterly staff meetings.
Maintains and increases professional knowledge and skills by attending training and staff development activities and keeping current by reading applicable professional literature.
Other duties as assigned.
Requirements:
Education:
A Bachelor's Degree in Human Service or a related field. Experience working with children within the last 5 years.
Good leadership skills.
Ability to work independently.
Good written and oral communication.
Good organizational and planning skills.
A safe driving record and CT license.
Ability to complete and be approved as a credentialed provider through Advanced Behavioral Health.
Per Diem As needed.
