• Conducting skilled nursing visits and coordinating comprehensive client care
• Serving as the main point of contact for all healthcare disciplines involved in patient care
• Managing the frequency of visits throughout the patient’s episode of care
• Consulting with physicians and providing detailed updates about patient care when necessary
• Collecting accurate OASIS data and maintaining current medication profiles
• Ensuring timely reporting of lab results to the physician
• Participating in weekly case conferences and monthly case manager meetings

• Current licensure as a Registered Nurse (RN) in the state of employment
• Preferred minimum of one year of clinical experience
• Demonstrated knowledge and skill in current nursing practices
• Valid state driver’s license and automobile liability insurance
