- Conducting skilled nursing visits and coordinating comprehensive client care
- Serving as the main point of contact for all healthcare disciplines involved in patient care
- Managing the frequency of visits throughout the patient’s episode of care
- Consulting with physicians and providing detailed updates about patient care when necessary
- Collecting accurate OASIS data and maintaining current medication profiles
- Ensuring timely reporting of lab results to the physician
- Participating in weekly case conferences and monthly case manager meetings
- Current licensure as a Registered Nurse (RN) in the state of employment
- Preferred minimum of one year of clinical experience
- Demonstrated knowledge and skill in current nursing practices
- Valid state driver’s license and automobile liability insurance
