Independent Contractor-Family Support Services

Health Connect America is seeking an Independent Contractor for Family Support Services to provide in home and community based support to children, adolescents, and families. In this role, you will deliver strengths based family coaching, parenting support, behavior management strategies, and skill building aligned with individualized care plans. You will collaborate with a multidisciplinary team, coordinate with schools and community agencies, complete timely documentation, and maintain professional boundaries.

This mission driven position suits contractors passionate about improving family stability, safety, and well being in their local communities.

Responsibilities

• Provide in-home and community-based family support and coaching to children, adolescents, and caregivers.
• Implement behavior management and parenting strategies based on individualized service or treatment plans.
• Coordinate care and communicate with therapists, case managers, schools, and community partners.
• Monitor family progress, identify needs and risks, and report concerns promptly to supervisors or clinical team.
• Complete accurate, timely documentation of visits, services, and outcomes per agency and payer standards.
• Educate families on community resources and assist with linkage to services and supports.
• Maintain professional boundaries, confidentiality, and compliance with ethical and regulatory requirements.
• Participate in team meetings, supervision, and trainings to support quality, safety, and professional growth.
