Our goal at Hollywood Home Health & Hospice Service is to create and implement a specific treatment plan for each patient which will be delivered at the convenience of their home environment. We endeavor to allow our patients to improve the quality of their health and live as independently as possible. HHHS is dedicated to providing home health services for individuals who demand the best care with a range of choices.

Primary functions are to administer skilled nursing care, under the supervision of a registered nurse, for patients in their place of residence, coordinate care with the interdisciplinary team, patient/family and Hollywood Home Health staff.

Candidate must have graduated from an accredited school of professional nursing. Current CA state license to practice as a Licensed Vocational Nurse in California.

• A minimum of one (1) year home health experience is strongly preferred.
• Must have an updated Physical and TB results.
• Valid CPR Certification.
• Nurse Liability Insurance (NSO)
• Must have a current valid Driver's License with automobile insurance in accordance with state and/or Agency requirements
• Works in patients home in various conditions; ability to work flexible schedule
• Bilingual is preferred but not required

Job Capabilities:

• Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients.
• Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition.
• Meet patient/family individualized psycho social needs. Requires hand-eye coordination and manual dexterity.
