Nurse Practitioner - Cognitive & Memory Disorders

Hoag Health, the top-ranked health system in Orange County, CA is seeking a Nurse Practitioner to join our growing Memory and Cognitive Disorders (MCD) program in Newport Beach, CA. • Provide outpatient specialty care for patients with dementia and related neurodegenerative conditions, including Alzheimer’s disease, frontotemporal dementia (FTD), Lewy body dementia, mild cognitive impairment, and other complex cognitive presentations
• Collaborate closely with Cognitive & Memory Physician and other specialists to deliver individualized evaluation and treatment plans
• Support patients and families with clear communication, counseling, and longitudinal care planning

• Nurse Practitioners being considered only
• 3-5 years of experience as a Nurse Practitioner managing Neurology or Geriatric patients
• Demonstrated experience managing dementia and neurodegenerative disease
• Graduate of an accredited Nurse Practitioner training program
• California Nurse Practitioner License (active and in good standing)
• Strong communication and interpersonal skills and a commitment to compassionate, family-centered care
• Ability to thrive in a multidisciplinary, team-based environment
