Wheeler Staffing Partners is seeking an experienced Territory Clinical Manager (RN) to lead and oversee clinical operations within a designated home health territory. This role is responsible for ensuring high-quality patient care, supervising clinical staff, and maintaining compliance with regulatory standards. The ideal candidate brings strong leadership experience in home health, a passion for patient-centered care, and the ability to manage clinical teams in a fast-paced, field-based environment.
Responsibilities- Provide clinical leadership and oversight for home health services within the assigned territory
- Supervise, mentor, and support clinical staff including RNs and field clinicians
- Ensure delivery of high-quality patient care in compliance with regulatory and organizational standards
- Monitor clinical performance, patient outcomes, and quality improvement initiatives
- Coordinate care plans and collaborate with interdisciplinary teams
- Manage scheduling, staffing, and productivity within the territory
- Conduct field visits and maintain visibility across service areas
- Ensure proper documentation, chart audits, and adherence to compliance requirements
- Support onboarding, training, and development of clinical team members
- Active Registered Nurse (RN) license in the state of California (required)
- Minimum 1 year of home health experience (required)
- Minimum 1 year of supervisory, management, or lead experience (required)
- 3–5 years of RN experience in home health, community, or clinical settings
- Experience managing clinical services and staff in a home-based care environment
- Strong leadership, communication, and organizational skills
- Ability to travel throughout the assigned territory
- Current RN licensure in California
- Valid CPR certification (must be maintained)
- Valid driver’s license and reliable, insured transportation
- Ability to commute within the assigned territory
