Hire Now Snapshot Hire Now Staffing is actively recruiting multiple seasoned, highly skilled Senior Home Health Clinical Care Coordination RN to join one of our valued client partners. This full-time direct hire opportunity is designed for a compassionate Registered Nurse who is passionate about delivering exceptional home health services while coordinating comprehensive patient care throughout the continuum of treatment. The successful candidate will serve as the primary clinical coordinator for an assigned patient caseload, providing skilled nursing care, developing individualized care plans, leading interdisciplinary collaboration, and ensuring compliance with home health regulations.
This role is ideal for an experienced RN who values patient advocacy, clinical excellence, and meaningful relationships with patients and their families. Multiple opportunities are available throughout Southern California.
Key Responsibilities
- Conduct comprehensive nursing assessments during admissions, resumptions of care, recertifications, routine visits, and patient discharges.
- Develop, implement, evaluate, and modify individualized Plans of Care in collaboration with physicians, patients, caregivers, and interdisciplinary team members.
- Complete accurate OASIS assessments supporting regulatory compliance, reimbursement, quality metrics, and optimal patient outcomes.
- Deliver skilled nursing services including wound care, medication management, IV therapy, injections, disease management, symptom management, and patient education.
- Educate patients and caregivers regarding medications, chronic disease management, safety practices, self-care techniques, and strategies that reduce hospital readmissions.
- Monitor patient progress while identifying changes in clinical condition and implementing timely interventions.
- Supervise Licensed Vocational Nurses (LVNs), home health aides, and other support staff to ensure coordinated, high-quality patient care.
- Participate in interdisciplinary case conferences and collaborate with physicians and healthcare professionals regarding treatment plans.
- Maintain accurate and timely electronic clinical documentation while complying with agency policies, Medicare regulations, OASIS standards, and payer requirements.
- Participate in quality improvement initiatives and assist in maintaining high clinical performance standards.
- Manage assigned patient schedules efficiently while responding to urgent patient needs and participating in on-call responsibilities as required.
- Graduate of an accredited school of professional nursing.
- Current unrestricted California Registered Nurse (RN) license.
- Minimum one year of Registered Nurse experience (one to two years preferred).
- Current CPR certification maintained throughout employment.
- Valid California driver's license with reliable, insured personal vehicle.
- Ability to travel throughout the assigned service territory.
- Strong clinical assessment, critical thinking, and patient education skills.
- Excellent documentation, organizational, communication, and time management abilities.
- Ability to work independently while collaborating effectively within an interdisciplinary healthcare team.

- Experience utilizing Electronic Medical Record (EMR) systems.

- Experience supervising LVNs, home health aides, or clinical support staff.
- Demonstrated ability to manage complex patient caseloads efficiently.
- Strong interdisciplinary collaboration and patient advocacy skills.
