Provider on PTO - Average Number of Patients Per Day: 0-1

Travel Reimbursement Information:
- ~BR50/night lodging cap
- All other bookings must be standard/economy
- Board Certified
- REQUIRED - Active VA License & DEA - REQUIRED
- BLS/ACLS Certified - REQUIRED
- COVID Vaccine - REQUIRED, noted at time of name clear
